LIABILITY WAIVER
A liability waiver will need to be filled out by each camper/staff and turned in during registration.

YOU MUST BRING THIS SIGNED LIABILITY WAIVER AND THE SIGNED PRE-CAMP SCREENING FORM TO CAMP REGISTRATION.
NEGATIVE COVID TEST OR VACCINATION
Prior to arrival, everyone attending camp (campers, leaders, volunteers) is required to have a negative COVID-19 test within 72 hours (3 days) of arrival, OR a fully completed vaccination per specific vaccine type recommendations. The Pfizer vaccine is now available for ages 12 and up.
Vaccine dates requiring a 2 part series must be complete two weeks prior to the start of their session.
Middle before June 6
Intermediate before June 20
Senior before July 4
Junior before July 18
FOUR-STEP HEALTH SCREEN
1. Screening at home before coming to camp, including a negative COVID-19 test (3 days prior to start of camp) OR completed vaccination (MUST SHOW PROOF). Campers and staff who have had a positive viral test in the 3 months prior to starting camp and have met the criteria to end isolation should have a letter from their healthcare provider documenting the positive test date and stating the individual is cleared to end isolation.

If someone is unable to meet the health standards on this form, we would ask that individual to stay home for the safety of all the others. If you are unsure whether you/they should attend camp, you can call our office, 715-284-0049 or contact the session director, prior to the first day of camp and we will work with you on the best plan possible.
2. We ask campers, their families, and camp staff to engage in physical distancing, mask-wearing, avoiding unnecessary travel, and refrain from indoor social gatherings with people outside of their households, 7 days prior to coming to camp.

4. Daily screening throughout your time at camp, which includes an antigen test performed as deemed necessary for individuals who are not fully vaccinated.
